A label showing the amount in the vial supplies the numerator. The independently confirmed total liquid volume supplies the denominator.

A dose is an amount of compound, usually expressed as mg or mcg. Concentration states how much compound is present in each milliliter. VialPilot never treats concentration as a dosing recommendation.
Display precision is not device precision
The app may display decimal values to explain the arithmetic. A physical syringe may not be capable of measuring every displayed fraction, and users should not infer more accuracy than the device and printed scale provide.
